29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) At the construction site at 32 Commerce Street, Montgomery, AL: On or about September 18, 2012, and at times prior to; employees were exposed to inhalation hazards from dust during the demolition and construction of interior walls and electrical wiring conduit. The employee(s) had not been provided training on Appendix D and were provided and wear dust mask respirators.
