29 CFR 1910.23(a)(5):     Infrequently used pit or trap door floor opening(s) with the cover(s) removed were not constantly attended by someone or protected on all exposed sides by removable standard railings:    Northeast corner of Tri-Flow Platform floor, northeast area of the workplace: On 09/20/2012, employees engaged in workplace activities were exposed to fall hazards through a floor opening that was not guarded or constantly attended by someone. The unguarded floor opening was 22 inches long, 14 inches wide, with a fall distance of 66 inches to the next lower level.

The drain pit did not have adequate guardrails to prevent a fall into the 16 foot deep pit.

The motors of all equipment being fueled shall be shut off during the fueling operation:    Diesel Fuel Storage Tank: fuel/maintenance storage site, northwest area of the workplace: On 09/20/2012, the employer did not ensure that storage tanks that contained flammable liquids used by employees to support workplace activities were posted with "No Smoking" signs. Employees were exposed to thermal burn injuries, from contact with flammable liquid fires initiated by employee smoking activities during refueling activities at flamable liquid storage areas.
