29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Worksite: Each employee using portable ladders was not provided with a training program to instruct and train each employee to recognize hazards related to ladders and in the procedures/means to minimize and/or eliminate those hazards. On or about September 17, 2012, the employee, using an aluminum extension ladder to perform painting task fell approximately 16 feet to the floor and sustained serious bodily injuries from the incident.
