29 CFR 1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one-eighth inch:    (a)  Integry Plastics, Inc., 10 Industrial Drive, Denver, PA - Employees used a Dewalt 6-inch bench grinder (# 756, Serial number 290338-YL0232) to remove clips on air hoses and were exposed to an impact/flying particle hazard when the space between the work rest and grinding wheel was three-quarters of an inch, on or about September 19, 2012.    No abatement certification or documentation required.

(b)  Integrity Plastics, Inc., 10 Industrial Way, Denver, PA - Two (2) 110-volt ungrounded pedestal fans contained energized outer coverings, thus exposing employees to possible electrical shock hazards, on or about September 19, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.23(c)(1):  Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides:    (a)  Integrity Plastics, Inc., 10 Industrial Drive, Denver, PA - Employees performing work activities above the tool room were exposed to an approximate 10.5 foot fall hazard when the midrail was missing on the guardrail system which surrounded the frontside (approximately 20.5 feet long) of the top of the tool room, on or about September 19, 2012. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
