29 CFR 1926.102(a)(1):  Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    This violation most recently occurred on September 20, 2012, where employees were exposed to struck-by hazards while operating pneumatic nail guns without the use of eye protection.    a.  Phase one of the Buckeye Trail Commons apartment complex, Buildings 6 and 7 at 6707 Buckeye Commons Way, Dallas, Texas 75215; and    b.  Phase two of the Buckeye Trail Commons apartment complex, Building 22 at 6717 Bexar Street, Dallas, Texas 75215.

29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    This violation most recently occurred on September 20, 2012, where employees were exposed to fall hazards while working approximately 18-feet above the ground level below without the use of a guardrail system or personal fall arrest system at a jobsite located at:    a.  Phase one of the Buckeye Trail Commons apartment complex, Buildings 6 and 7 at 6707 Buckeye Commons Way, Dallas, Texas 75215; and    b.  Phase two of the Buckeye Trail Commons apartment complex, Building 22 at 6717 Bexar Street, Dallas, Texas 75215.

29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     This violation most recently occurred on September 20, 2012, where employees were exposed to   fall hazards while using an extension ladder to access the upper landing that did not extend at least three feet above the landing.    a.
