29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:       On or about 9/16/12, employees were exposed to a fall from height of approximately 20ft. while working at the edge of a low slope roof at the commercial building at West IH 10 & Presidio Pwky preforming roofing work.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:       On or about 9/16/12, employees were exposed to a fall from height of approximately 10 ft, while using a fiberglass A frame ladder to gain access to the roof of the commercial building with its rails not extended 3ft. above the landing at West IH 10 $ Presidio Pkwy.
