29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Mountain Roofing & Repair, Inc at 331 Nottingham Rd, Avon, 81620: On August 24, 2012 the exposing employer did not ensure that employees were protected from injury, in that two employees removing shingles from a 5.5-in-12 pitch roof from a three-story residential apartment complex were not utilizing a fall protection system. The two employees were wearing fall harnesses; however, they were not connected to the rope grab system in place. This condition exposed employees to a fall hazard of approximately 30 feet.
