29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Jobsite: 3437 D Street, mattress upholstery machine - On or about 9/21/2012, an employee performing maintenance and lubrication on a mattress upholstery machine was exposed to unexpected energizing or release of stored energy without an energy control procedure, employee training and periodic inspections. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Jobsite: 3437 D Street, waste baler - On or about 9/21/2012, an employee was exposed to unexpected baler operation and crushing hazards while working in the baler with the baler door open and the baler door shutdown interlock removed and inoperative. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
