29 CFR 1926.602(d): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely as demonstrated by the successful completion of training and evaluation specified in CFR 1910.178(l): (a) 534 Shrewsbury Commons, York PA - An employee was operating the Gradall G6-42A rough terrain forklift and did not receive a combination of formal instruction, practical training, and an evaluation of the operator's performance in the work environment, on or about September 17, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
