29 CFR 1910.23(c)(3):     Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board.    a)  On or about August 14, 2012, and times prior thereto, at the Flowers Bakery in Thomasville, GA, the employer failed to ensure that a walkway in the fermentation box adjacent to moving dough troughs was guarded with a standard railing and toe board, exposing its own employees, and sanitation employees to struck-by and crush-by hazards.    ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.263(i)(2):     Fermentation room doors did not have nonshatterable wire glass or plastic panels for vision through doors:    a)  On or about August 14, 2012, and times prior thereto, at the Flowers Bakery in Thomasville, GA, the employer failed to ensure that employees using a walkway in the fermentation box adjacent to moving dough troughs were protected by non-shatterable wire glass or plastic see-through panels on access doors, exposing its own and sanitation employees to serious injuries resulting from struck-by and crush-by hazards.    ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.263(i)(7)(iii):     A sufficient number of stop buttons was not provided to enable quick stopping of conveyor(s) where the hazard of getting caught existed:    a)  On or about August 14, 2012, and times prior thereto, at the Flowers Bakery in Thomasville, GA, the employer failed to ensure that employees using a walkway in the fermentation box adjacent to moving dough troughs were protected by electronic sensors on access doors to enable quick stopping of the conveyor system, exposing employees to serious injuries resulting from struck-by and crush-by hazards.    ABATEMENT CERTIFICATION IS NOT REQUIRED.
