29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 09/12/12, employees were exposed to a fall from height hazard of approximately 15 feet to the concrete ground below, while using an aerial lift to paint graphic art on the exterior of a building.
