29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 273 Broad Street, Manchester, CT. - On or about 9-20-12 - Employees were using an 8 foot aluminmum straight ladder to access and exit a 6 to 7 foot deep excavation. The ladder did not extend 3 feet above the asphalt roadway exposing employees to 6 foot fall hazard.
