29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:    a)  Employees working from two-point adjustable suspension scaffolds were not protected from fall hazards of approximately 50 feet to the ground by a personal fall arrest system.    No abatement certification or documentation is required for this item.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    a)  Employer did not provide training to each employee in order to recognize hazards associated with working from two-point adjustable suspension scaffold and to understand procedures to control or minimize those hazards.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:    a)The employer's safety and health program was ineffective as to fall protection training. On September 17, 2012, an employee was not protected from fall hazards of approximately 50 feet to the ground while walking and working along unprotected edges of the roof.
