29 CFR 1910.134(e)(1):     The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a.) Buckeye Launderers & Cleaners, LLC located at 1805 Oakwood Napoleon, OH; On or about September 20, 2012, the employer did not have medical evaluations completed for employees who had half-mask tight-fitting respirators available to them for required use. Employees worked with various chemicals, such as, but not limited to, sodium hydroxide and acetic acid.

Buckeye Launderers & Cleaners, LLC located at 1805 Oakwood Napoleon, OH; On or about September 20, 2012, the employer did not assure suitable facilities for quick drenching or flushing of eyes and body were in working order in the Chemical Storage Area.  Employees were exposed to burn injuries while working with corrosive materials, such as TurboPower Plus and Advacare 120 Sanitizer laundry chemicals with a broken eyewash/shower and a blocked sink in case of emergency.
