29 CFR 1926.503(a)(2)(iii):  The employer did not ensure that each employee was trained as necessary by a competent person in the use and operation of personal fall arrest systems:    a) Ridge View Building 1 - On or about 19 September, 2012 - Employees were installing roof trusses on the building without properly wearing attached personal fall protection.  Employees were exposed to a fall hazard of 30 feet.     b) Ridge View Building 1 - On or about 19 September, 2012 - Employees were installing roof trusses on the building and not enough anchor points were installed to protect each employee. Employees were exposed to a fall hazard of 30 feet.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:Note:  29 CFR 1926.1052(c)(7) applies when the top edge of a stairrail system also serves as a handrail.    a) Ridge View Building 1 - On or about 19 September, 2012 - Employees were utilizing a stairway to access the first through third floors that was not equipped with at least one handrail. Employees were exposed to a fall hazard of 12 feet.
