29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift and after any occurrence which could affect a scaffolds structural integrity: (a) Osuna Remodeling @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the scaffolding had not been inspected by the exposing employer before each shift to identify defects. Defects observed include, but not limited to, missing guardrails, missing planking, and missing bracing where employees were straddling and/or walking. This condition exposed employees to an approximate fall hazard of 12ft.

29 CFR 1926.451(b)(1): The employer did not ensure that all working levels of the scaffold was fully planked or decked between the front uprights and the guardrail support: (a) Osuna Remodeling @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the exposing employer did not ensure that the working level on the fabricated frame scaffold was fully planked. The second level, on the east-facing scaffold, was equipped with 2, rather than 3, 2x10in wooden planks. An employee applied plaster from the second level of the scaffold and was exposed to an approximate fall hazard of 12ft. (b) Osuna Remodeling @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the exposing employer did not ensure that the working level on the fabricated frame scaffold was fully planked. The first level, on the east-facing scaffold, was equipped with 2, rather than 3, 2x10in wooden planks.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar structure was not used: (a) Osuna Remodeling @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the exposing employer did not equip the scaffold with ladder access. An employee accessed the scaffold from an extension ladder located on a porch roof. The employee traveled from the porch roof on to the scaffold system. This condition exposed the employee to an approximate fall hazard of 12ft.
