29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: On or about September 12th, 2012 at worksite 421 N. Stadium Boulevard in Columbia, Missouri- Employee was performing work operations from a mobile scaffold that was hoisted by a SkyTrak fork truck. The attachment that the employee was working from was not approved by the manufacturer. Abatement certification for this violation is not required.
