29 CFR 1926.451(b)(1): The employer did not ensure that all working levels of the scaffold was fully planked or decked between the front uprights and the guardrail support: (a) Red Hill Construction, Inc. @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the controlling employer did not ensure that the working level on the fabricated frame scaffold was fully planked. The second level, on the east-facing scaffold, was equipped with 2, rather than 3, 2x10in wooden planks. An exposed employee applied plaster from the second level of the scaffold and was exposed to an approximate fall hazard of 12ft. (b) Red Hill Construction, Inc. @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the controlling employer did not ensure that the working level on the fabricated frame scaffold was fully planked. The first level, on the east-facing scaffold, was equipped with 2, rather than 3, 2x10in wooden planks.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented: (a) Red Hill Construction, Inc. @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the controlling employer did not ensure that fall protection systems were utilized by employees engaged in stucco application. Two employees applied stucco while on top of a porch roof. This condition exposed employees to an approximate fall hazard of 12ft. (b) Red Hill Construction, Inc. @ 2481 S. Lincoln St., Denver, CO 80210: On 9/20/12, the controlling did not ensure that employees engaged in stucco application were protected from a fall. Employees used the porch roof for access to the fabricated frame scaffold. This condition exposed employees to an approximate fall hazard of 12ft.
