29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a) 100 Montcalm Street - On or about 19 September, 2012 - Employees were installing roofing shingles on a residential home without the use of conventional fall protection. Employees were exposed to a fall hazard of 11 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    a) 100 Montcalm Street - On or about 19 September, 2012 - Employees were using a ladder to access the scaffolding landing that did not extend a minimum of 3 feet above the landing. Employees were exposed to a fall hazard of 6 feet 5 inches.
