29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to the items of this section: Jobsite located at 1400 West Bond Circle, Lincoln, NE: Lockout/tagout procedures did not meet the requirements outlined above. Specifically, the procedure for Pin Inserter #2 did not accurately depict the methods for removing pneumatic pressure from the machine. The machine had been modified 6-8 months prior to this inspection and the procedure was not updated. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Jobsite located at 1400 West Bond Circle, Lincoln, NE: A Pin Inserting machine is not effectively guarded to prevent contact with the moving parts of the machine. Specifically, a light curtain used to guard the moving parts that create nip points can be disabled by the operator rendering the guarding ineffective. On August 2, 2012, an employee disabled the guard to lean into the area to observe pin placement and had the tip of his left index finger amputated. Disabling the light curtain is a normal practice allowing the operator to closely observe the pin insertion. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
