29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a) 15 Windover Drive, Worksite - On or about September 17, 2012, employees were using pneumatically powered nail guns to conduct frame carpentry work, without any means of eye protection, subjecting them to eye trauma from fugitive debris produced during nail gun activation.

29 CFR 1926.451(c)(2)(v):     Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    a) 15 Windover Drive, Worksite - On or about September 17, 2012, an employee is performing finish carpentry work while being lifted no less than 22 feet above grade by a Pettibone Variable Reach Rough Terrain Forklift while standing on an unsecured platform.
