Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On September 21, 2012, at a workplace located at 5757 North Jim Miller Road, Dallas, Texas; employees doing roofing work with no fall protection were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On September 21, 2012, at an establishment located at 5757 North Jim Miller Road, Dallas, Texas; on the west and rear sides of the building employees using portable extension ladders and accessing roof areas were exposed to a fall hazard. The ladders side rails did not extend at least three feet above the upper landing surface.
