On or about September 19, 2012 - at the above addressed jobsite, an employee working from the top of a parts machine engaged in rigging operations, was exposed to falls of approximately ten feet eight inches (10'8") due to the lack of guardrails on all open sides.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a. On or about September 18, 2012 - at the above addressed jobsite, employees were operating powered industrial trucks during the breakdown of a show. Sufficient safe clearences were not allowed for aisles and where ever turns must be made, thereby exposing employees to struck by hazards.
