29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    a) Pier 1 Imports/Staten Island, NY store - Storage Area: Boxes of different sizes which contain material such as but not limited to home decor items were stacked in an unstable condition exposing employees to being struck by falling boxes.    Condition observed on or about September 21, 2012.

29 CFR 1910.176(c): Storage areas were not kept free from accumulation of materials that constituted hazards from tripping, fire, explosion or pest harborage:    a) Pier 1 Imports/Staten Island, NY store - Storage Area: Boxes of different sizes which contain material such as but not limited to home decor items were stored throughout the back room area, creating trip and fire hazards.    Condition observed on or about September 21, 2012.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Pier 1 Imports/Staten Island, NY store - Chemical Storage Area: The employer did not provide employees with effective information and training on hazardous chemicals used for cleaning in their work area at the time of their initial assignment, and whenever a new physical or health hazard was introduced into their work area. Employees are exposed to chemicals, including but not limited to hydrogen peroxide contained in Hydrogen Peroxide GP Cleaner, etc.      Condition observed on or about September 21, 2012.

29 CFR 1910.37(a)(3): Exit routes were not continuously maintained free of obstructions or impediments to full instant use in the case of fire or other emergency:     a) Pier 1 Imports - Staten Island, NY -Stock Room/Back Room Area: The emergency exit route was blocked by storage material preventing free access to the exit:    1) Access to Exit door in the stock room was blocked by boxes filled with merchandise.  2) Access to Exit door in the back room by chemical storage was blocked by merchandise stored in the hallway in addition to the shovel and a vacuum stored in front of the exit door.     Condition observed on or about September 21, 2012.    Pier 1 Imports, was previously cited for violation of this Occupational Safety and Health standard or its equivalent standards, 29 CFR 1910.37(a)(3) which was contained in OSHA Inspection Number 315486498, Citation # 1, Item #1, and was affirmed as a final order on May 20, 2011, with respect to the workplace located at 145 Great Road, Acton, MA 01720.

29 CFR 1910.303(g)(1)(ii):  Working space about electric equipment rated 600 volts, nominal, or less was used for storage:    a) Pier 1 Imports - Staten Island, NY - Storage Room Area: Electrical panel box rated 600volts, or less was not maintained free of storage. Boxes filled with merchandise were placed in front of electrical panels located throughout the storage/stock room. The access to the panels was blocked.    Condition observed on or about September 21, 2012.    Pier 1 Imports, was previously cited for violation of this Occupational Safety and Health standard or its equivalent standards, 29 CFR 1910.303(g)(1)(ii) which was contained in OSHA Inspection Number 315486498, Citation # 1, Item #2, and was affirmed as a final order on May 20, 2011, with respect to the workplace located at 145 Great Road, Acton, MA 01720.
