29 CFR 1926.451(g)(4)(ii):  The top edge height on guardrail systems on supported scaffolds manufactured or placed in service after January 1, 2000 were not installed between 38 inches (0.97 m) and 45 inches (1.2 m) above the platform surface:  (a) Gym Area Renovation, North Side of Building - Employees placed a metal personnel platform on the forks of a Pettibone Traverse lift, model 8044, serial number 4587.  An employee worked from this metal personnel platform, at heights of 13 feet 5 inches to 17 feet above the ground, pressure washing the exterior of the north wall of the Gym Area Renovation.  The top rail of the guardrail system along the rear of the personnel platform had a height above the platform surface of 36.5 inches because the vertical support post had gotten bent and cracked and the top rail had also gotten damaged, on or about September 21, 2012.

An employee worked from this metal personnel platform, at heights of 13 feet 5 inches to 17 feet above the ground, pressure washing the exterior of the north wall of the Gym Area Renovation.  The employees did not have the personnel platform attached to the forks of the lift.  Additionally, the operator of the forklift moved the forklift horizontally while the employee working from the personnel platform occupied the platform, on or about September 21, 2012.  ZIMMERMAN & HERR MASON CONTRACTORS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.451(c)(2)(v), WHICH WAS CONTAINED IN:  OSHA INSPECTION NUMBER 314242421, CITATION NUMBER 1, ITEM NUMBER 1b, AND WAS AFFIRMED AS A FINAL ORDER ON JANUARY 11, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT GRINGS HILL ROAD INTERCHANGE ROUTE 222, MOHNTON, PENNSYLVANIA  19540.
