29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employee exposure. Where occupational exposure remains after institution of these controls, personal protective equipment shall also be used.    a. Medical Side  An employee used EXEL International 5 ml Syringe and 22G x 1  inch Needle Combination (26219) to administer intramuscular injections of Cyanocobalmin (Vitamin B12), Solu Medrol, Gentamicin, and Ceftriaxone to patients. These needles were not equipped with built-in safety features designed either to eliminate or to minimize the risk of accidental percutaneous injuries from needles contaminated with bloodborne pathogens, such as, human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV).  Observed On or About September 21, 2012    b. Medical Side  An employee used Exel International 3 ml Syringe with 25G x 5/8 inch Needle Combination (26100) to administer intramuscular injections of Tetanus and Diphtheria Toxoids Adsorbed vaccines to patients. These needles were not equipped with built-in safety features designed either to eliminate or to minimize the risk of accidental percutaneous injuries from needles contaminated with bloodborne pathogens, such as, human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV).  Observed On or About September 21, 2012    c. Dental Side  The Dentists used Covidien Monoject 400 Plastic Hub Dental Needles 30G Short (8881400074) and Covidien Monoject 400 Plastic Hub Dental Needles 25G Long (8881400033) to administer Lidocaine, an anesthetic, to patients. These needles were not equipped with built-in safety features designed either to eliminate or to minimize the risk of accidental percutaneous injuries from needles contaminated with bloodborne pathogens, such as, human immunodeficiency virus (HIV), hepatitis B (HBV), and hepatitis C (HCV).  Observed On or About September 21, 2012

Medical Clinic, Examination Room, Biohazard Trash Container - The employer did not place regulated waste, excluding contaminated sharps, in the appropriate container. Employees conducted medical procedures, such as, blood collection.  In the event of the container being tipped or upended, regulated waste can spill onto the floor.
