29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed: (a) First Floor of Gym Area Renovation, Bottom of New South Stairway - An employee did electrical work in a hole dug in the floor. The hole had an opening of 6 feet 6 inches by 5 feet 5 inches and a depth of 48 inches. The employee who worked in the hole used a 10-foot step ladder leaned in a closed position to get into and out of the hole, on or about September 21, 2012.
