OSH ACT of 1970 Section (5)(a)(1):       The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being stuck-by materials and equipment in the event of mechanical failure:     On or about September 18, 2012, and times prior thereto, on J-Avant Ranch, J-Avant Unit 1 LASC 3H, the employer did not ensure pressure relief was provided to fracturing pumps and related piping to eliminate excessive system pressure or explosions.  Employees authorized to work in high-pressure danger zones to service equipment on multi-pump systems, such as, but not limited to, block valves, pumps, manifolds and iron, were not protected from build-up of high pressure.
