29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: This violation occurred on September 19, 2012, at a workplace located at 5353 Las Colinas Blvd., Irving, TX 75039, Building #7; where at least four (4) employees, who were working from the rafters of two-story apartment complex and were not protected from falling to the ground below. ANTONIO BANDA/BANDA CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312121106, CITATION NUMBER 1, ITEM NUMBER 2, ISSUED ON 05/25/2009, AND WAS AFFIRMED AS A FINAL ORDER ON 06/08/2009, WITH RESPECT TO A WORKPLACE LOCATED AT 6044 EAST LOVERS LANE, DALLAS, TX 75227.
