29 CFR 1926.651(c)(2):     A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    a)  The employer does not ensure that a stairway, ladder, ramp or other safe means of egress is located in excavations that were four (4) feet or more in depth.  This violation occurred on or about September 20, 2012 when employees were allowed to walk down a five (5) foot embankment to access the work area inside an excavation where hazards such as roadways, mechanical equipment, and unstable soil were present.
