29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: On or about 03/31/2012, the storage rack located in the middle of the warehouse was not braced to make it stable. Employees stacking on and or retrieving heavy furniture susch as sofas the rack were exposed to crush injuries

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: In the warehouse a fork truck with a broken glass shield was not removed form operatiion or repaird. The employee were exposed to laceration and cut injuries from potaentially shattering glass.

29 CFR 1910.178(g)(2): Battery changing and charging installations for industrial trucks were not provided with facilities for flushing and neutralizing spilled electrolyte: In the warehouse where battery charging is conducted at the west wall, spill nutralizing and or flushing apparatus was not provided. The employees were exposed to inhalation hazard.
