29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) Exterior, Southside Main Entrance to Residential Lobby - A foreman was observed using a Stihl gasoline powered chop saw to cut into existing limestone and not wearing eye protection.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    a) Exterior, Southside Main Entrance to Residential Lobby - Laborer and foreman were observed standing on a scaffold work platform, 11 feet, 7 inches above the ground while cutting out a limestone window jamb, and the back of the work platform was 13 inches from the rear upright of the scaffold frame.
