29 CFR 1910.178(g)(2): Battery changing and charging installations for industrial trucks were not provided with facilities for flushing and neutralizing spilled electrolyte:  a)  Mailroom, Battery Charging Area - On or about September 13, 2012, where approximately 40 forklift battery charging stations are located, the spill kit was unmarked and blocked by pallets, effectively making the the flushing and neutralizing capabilities unavailable in the event of a spill.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a)  Mail Room - On or about October 3, 2012, a relocatable power tap connected in tandem with an extension cord plugged into a dual receptacle outlet was used to provide power to appliances to include but not limited to a refrigerator and coffee maker.  b)  Engineering Services, Carpentry Area - On or about October 3, 2012, a cut-off saw was powered by a flexible cord plugged into a shop-made extension cord which was made from parts used in a manner in which they were not manufactured in that, the attachment plug was a quad junction box intended for permanent installation.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv).    (a)  Machine Shop - On or about September 13, 2012, employees cut metal track with the Dake Cut-Off Saw where the flexible power cord was used as a substitute for the fixed wiring, in that it did not have an attachment plug and was run through a hole in floor.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a)  Engineering Services - On or about October 3, 2012, two electrical panel boxes were blocked in that paper and other combustible materials were piled on top of a desk located in front of and below the electrical cabinets.

29 CFR 1910.179(j)(2)(iii): Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed:  a) Baler Area - On or about 09/13/2012, employees were using a 1/2 Ton Chain Hoist to load rolls of baling wire onto the baler with a damaged hook, in that the hook was twisted and the throat opening was expanded, not allowing the safety latch to contact the tip of the hook.
