29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    A.  Vanderwerff & May Builders, Inc. worksite located at 12015 St. Rt. 65, Ottawa, Ohio: On or about September 18, 2012, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns to secure sheets of plywood. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners on the east side of the commercial structure.

Vanderwerff & May Builders, Inc. worksite located at 12015 St. Rt. 65, Ottawa, Ohio: On or about September 18, 2012, the employer did not ensure the 20ft x 4ft platform was securely attached to the forks. The unsecured platform was at a height of 15ft.

Vanderwerff & May Builders, Inc. worksite located at 12015 St. Rt. 65, Ottawa, Ohio: On or about September 18, 2012, the employer did not ensure employees working on the 20ft x 4ft platform were protected from fall hazards while performing framing activities. The platform was not equipped with fall protection on the ends exposing employees to a fall distance of approximately 15.00 feet.
