29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    a) Northeast Corner of the Building, Second Floor - A laborer was observed without fall protection while at a window  throwing debris outside into a dumpster exposing him to a 17 foot 9 inch fall hazard.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a) East Side of the Building , Adjacent to the Alley - Stairways were not equipped with at least one handrail. A superintendent was observed entering the basement of the building using the concrete stairway which was not equipped with a standard handrail exposing him to a 6 foot 4 inch slip, trip, and fall hazard.

29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof:    a) Throughout the Building - A superintendent, foremen and workers were observed working in a four story building under renovation where fire extinguishers were not provided exposing the employees to a smoke and fire hazard.
