29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a. On or about August 29, 2012, in the laundry room, the housekeepers were exposed to corrosive chemicals such as, but not limited to, Disinfecting Heavy Duty Acid Bathroom Cleaner Maxx Dual Action Floor Cleaner- Bright Speed, and Oasis Pro 64 Toilet Bowl Cleaner while replacing the empty bags from the wall-mounted Ecolab system with new ones.  The employer provided an eye irrigation system that had the irrigation solution expired since April 2012, and the wall-mounted irrigation system did not provided a 15-minute continuous flushing of the eyes in case of an emergency.    b. On or about August 29, 2012, in the laundry room, the Care Givers were exposed to corrosive chemicals such as, but not limited to, Tri-Star Oxi-Brite, Tri-Star L2000 XP, and Aqua Soft 2000 laundry detergents while changing the empty drums with new ones. The employer provided an eye irrigation system that had the irrigation solution expired since April 2012, and the wall-mounted irrigation system did not provided a 15-minute continuous flushing of the eyes in case of an emergency.

29 CFR 1910.134(k)(6): The employer shall provide the basic advisory information on respirators, as presented in Appendix D of this section, in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer:    a. On or about August 28, 2012, maintenance employees were provided with a N95 filtering face piece to use on a voluntary basis while using paints and conducting general cleaning and the employer did not provided the information contained in the Appendix D of the respiratory protection standard.    b. On or about August 28, 2012, housekeepers provided with a single strap filtering face piece to use on a voluntary basis while cleaning resident rooms the employer did not provided the information contained in the Appendix D of the respiratory protection standard.
