29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) Woodson's Exterior Remodeling, Perrysburg, Ohio:  On or about September 17, 2012, at worksite adjacent to 25571 Seminary Road, Perrysburg, Ohio, the employer failed to ensure that each employee used appropriate eye protection while engaged in nailing activities, such as securing shingles to the structure roof with a pneumatic nailer, exposing employees to eye injuries from flying particles.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Woodson's Exterior Remodeling, Perrysburg, Ohio:  On or about September 17, 2012, at worksite adjacent to 25571 Seminary Road, Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while applying new roofing materials to a steep sloped roof on a residential type structure.  Employees were exposed to a potential fall of approximately 20.2 feet to the ground below.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Woodson's Exterior Remodeling, Perrysburg, Ohio:  On or about September 17, 2012, at worksite adjacent to 25571 Seminary Road, Perrysburg, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems.  Employees were exposed to falls of approximately 20.2 feet to the ground below, which could result in permanent disability or death.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Woodson's Exterior Remodeling, Perrysburg, Ohio:  On or about September 17, 2012, at worksite adjacent to 25571 Seminary Road, Perrysburg, Ohio, the employer did not ensure that the side rails of three aluminum extension ladders were extended at least three feet above the landing.  Employees were exposed to falls while using the ladders for access to the working level inorder to perform roofing activities.
