29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. a) On or about September 19, 2012; on the east side of the jobsite, employees were allowed to work near scrap lumber with protruding nails. The scrap lumber with protruding nails was staged adjacent to the employees work area and next to a passageway that employees used to access the rear of the jobsite.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports: a) On or about September 19, 2012; on the west side of the jobsite, employees were allowed to work from tubular welded scaffolding that was not fully planked. Employees were observed sitting on a scaffold where only one scaffold plank was installed at a height of approximately seven feet while installing supports under an elevated building.

Employees were observed sitting on a scaffold that did not have base plates installed on the scaffold legs at a height of approximately seven feet while installing supports under an elevated building.
