Covers and/or guardrails shall be provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.    Vertical Mill Area - On or about September 17, 2012, the employer failed to provide a cover and or guardrail to protect employees from falling into an open pit in the vertical mill area that measured 60 inches deep, thereby exposing employees to fall hazards.
