On or about 09/21/12, on lot #134, Gardere lane the employer failed to ensurethat the employee(s) worked from a scaffold platform at least 18 inches wide. The employees were working from planks 6-10 inches wide at elevations of 10-17ft. above the ground.

On or about 09/21/12, on lot #134, Gardere Lane the employer failed to provide an adequately built scaffold for the employees to use during the house construction project. The scaffold did not have top rails, mid-rails or end rails to protect the employees from a fall from height.
