29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a) On or about September 20, 2012; at the center of the jobsite, employees were allowed to utilize an extension cord set to power a compound miter saw without visually inspecting the cord before each days use for defects. The extension cord set was found to be missing a ground pin.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a) On or about September 20, 2012; at the center of the jobsite, employees were allowed to utilize an extension cord that was missing a ground pin. The extension cord was used to power a compound miter saw used to cut PVC pipe. The missing ground pin prevented the extension cord from having a continuous path to ground.
