29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Building 9 - On or about September 20, 2012, an employee was working on a tubular frame scaffold, no less than 12 feet above grade, and was throwing sections of decorative stone to ground workers who were not protected by wearing the appropriate head protection.
