29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: At and around the fabrication area, employees were exposed to slips, trips, and falls: A. 5/8 inch diameter wire rope eyes were not secured to the timber work platforms. B. Scrap, thick, plastic sheeting and cracked plywood sheeting was left in and around the work area. C. A long, shallow depression was traversed by employees as they worked and traveled around the fabrication area.
