29 CFR 1910.178(l)(1)(i): The employer did not ensure each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about September 20, 2012, and times prior thereto, at Atlantic Corporation of Wilmington, Inc., who occupies space within the Flanders Corporation facility, located in Ardmore, Oklahoma, Atlantic Corporation of Wilmington, Inc. did not ensure an employee operating a Toyota forklift truck, model 5FBE15, S/N 5FBE15-31145, was competent in operating the forklift and was certified. This exposes employee(s) to struck-by, caught-in and/or crushed-by hazards.
