Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: location: 250 Main St., northwest corner on or about: 9/21/12 a) An employee, dropping off plywood to the site, climbed over a guardrail and walked on top of the concrete foundation wall. The employee, who was not provided with any means of fall protection, was exposed to a fall of 24 feet.
