29 CFR 1910.27(f): "Maintenance." All ladders shall be maintained in a safe condition. All ladders shall be inspected regularly, with the intervals between inspections being determined by use and exposure. (a) The cage installed on the ladder for Bin 4 had a damaged vertical bar exposing employees to potential impalement hazards. (b) The cage installed on the ladder for Bin 3 had a damaged vertical bar exposing employees to potential impalement hazards. (c) The cage installed on the ladder for Bin 1 had a damaged vertical bar exposing employees to potential impalement hazards.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) When performing group lockout, each authorized employee was not placing a lock on each designated energy control devices. One lock was applied when more than one person was performing servicing/maintenance on a piece of equipment. This exposed employees to accidental/inadvertent restart of equipment.
