OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and explosion hazards: On or about September 23, 2012, Pioneer Rig #61, employees were exposed to struck-by and explosion hazards when the Ariel booster unit was not operated in accordance with the Operator's Manual. A useful and feasible means of abatement, among others, would be to ensure that the equipment is operated and maintained in accordance with all operating instructions contained in the operator/maintenance manual.
