29 CFR 1910.147(c)(7)(i)(A):     Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.      a. At the workplace, the employer did not provide adequate authorized employee training to employees who operated the Spinelli Tube Mill located near the south end of the warehouse to ensure specific procedures were followed for lock-out/tag-out during set-up operations, exposing employees to an amputation hazard.    On or about 9/24/2012, two employees were setting-up a Spinelli Tube Mill for broom handle production.  An employee checked the handles to make sure the handles were the appropriate length before the product run.  The employee was able to place his hands into the machine while it was running approximately 5 times to retrieve the handles to check their length.

29 CFR 1910.178(l)(5):     Avoidance of duplicative training. If an operator has previously received training in a topic specified in paragraph (l)(3) of this section, and such training is appropriate to the truck and working conditions encountered, additional training in that topic is not required if the operator has been evaluated and found competent to operate the truck safely.      a. At the workplace, the employer did not ensure employees who had previous forklift training were re-evaluated for competency before being allowed to operate a forklift.
