29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.      a) On or about September 4, 2012; second floor near stairs; employee was working off a scaffold platform without a personal falll arrest system in place to prevent him from falling 13 feet to the lower platform.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.       a) On or about September 4, 2012; top of second floor; employees working on the top plate and beams of the residential home were not protected from falling 11 feet to the lower platform or 25 feet to ground level by a personal fall arrest system while installing the plywood floor.     b) On or about September 4, 2012; second floor; employees were exposed to a fall hazard of 10 feet while working near wall openings that were not protected by a top rail    c) On or about September 4, 2012; second floor; employees were accessing the upper floor via stairs that were not equipped with railing exposing the employees to a fall to the lower platforms; 6 feet and 10 feet.

29 CFR 1926.1053(b)(1):     When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    a) On or about September 4, 2012; second floor; employees were using a six foot step ladder to access the roof/working platform; ladder was an estimated eight feet short to be place three feet above the platform.

The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.    a) On or about September 4, 2012; employees were using an extension cord to power portable electrical equipment, such as but not limited to circular saws, which had the ground pin missing.
