29 CFR 1926.416(a)(1):     Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    On or about 09/19/2012, employees were performing construction activities in close proximity to the high voltage power line running parallel to the eave of the structure whereas neither individual where protected against electrical shock. The employees were within 10 feet of the power line whereby a contact incident occurred resulting in a fall.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    On or about 09/19/2012, employees emplacing the Werner 32 foot extension ladder were exposed to electrical hazards whereby the ladder was set up approximately 12 inches from the live overhead power lines.
